Required Documentation

To import a small quantity of drugs for personal use, the following documents are required:
• Valid doctor’s prescription
• Patient ID proofs
• Form 12B (Import Permit)
• Customs Duty Exemption Certificate (CDEC) [Applicable only for life-saving drugs]
• Duty Bond (if requested by customs)
• Authorization Letter (CHA/Forwarder)
• Delivery Order (DO) Collection Letter
Note: All documents must be self-attested by the patient. If the patient is a minor, a parent can sign the documents. In this case, the signed person’s ID proofs (PAN and Aadhar copy) are required.
